Via Instapundit, it turns out that the Genesis spacecraft crashed, because the two sensors that would have told the ship that it was time to deploy the parachute were installed backwards -- by design!

Not a good design, mind you...

That's the trouble with spacecraft design. When the system crashes, it crashes.
